Made an appointment. Upon arrival I was informed that Peak View Optometry required disclosure of my full social security number. VSP, my vision insurance provider, states that providing the last 4 social security numbers is adequate to receive service and authorization for insurance purposes. When I asked why providing a full social security number was required, Peak View Optometry mentioned that it is office policy without providing a justification for this office policy. I do not recall having to provide my full social security number to receive optometry services before in prior cities I have lived at and optometrists I have received service from. At the very least, Peak View Optometry should have informed me of this office policy prior to scheduling an appointment with me. I would prefer to not disclose such identifying and delicate, personal information.

Perfect place for eye glass repair! I lost a screw on my frames, and the place I bought them from said they couldn't help me. I would need new frames. I stumbled upon this place while driving by, so I gave it a shot. So glad I did! Not only was he able to help me, but I was in and out of there in 5 min for less than the price of a coffee.
